Harsh change into 4th gear

Hi everyone, 

Had a Grand Voyager years ago and recently bought another one due to expanding family but it developed a slight issue. Its a 2005 2.8 CRD and happily goes from first to 3rd while accelerating but when i get to 30-40 and drop off to cruise it changes quite harshly into 4th and feels like it’s going to stall then gradually eases until I try accelerate again. For a brief moment it feels like it’s going to stall but then changes down to 3rd and all happy again. Anyone have and ideas of what could be the issue?

Most likely sounds like an internal Gearbox problem. Key question is how many miles has it done and how good has the servicing been i.e has the Gear oil ever been changed ?.
